We are an independent practice, not owned by a hospital.
Our main office is located in the Proctor Professional Building at Proctor Hospital in Peoria, IL.  We have an additional office in Morton, IL.

Of note:
Rescheduling - As a courtesy to other patients, we request that office visit rescheduling or cancellations be called into the office at least 24 hours in advance at (309) 692-1700.

Billing questions – Please direct to our Billing Department at (309) 671-8749, Monday - Friday, 8am – 4pm.


What to bring to your appointment:

  • Insurance card(s)
  • Drivers license or other photo identification
  • List of current medications and dosages
  • List of any allergies to medicines
  • Pertinent medical records or test results
  • Co-pay, if applicable
  • If you are a new patient, or there has been a change in your medical history since
    your last visit, please fill out a new “patient information form” and bring with you.
New Patients:
You may save time by printing and filling out the new “patient information form” prior to your visit.   Click on the link, print out, and complete.  Just hand in on arrival.  If you prefer to fill out the form in our office, please plan to arrive 15 minutes early to complete the paperwork.

Privacy:
Patient privacy is very important to us.  Our procedures and practices meet or exceed those of the Health Insurance Portability and Accountability Act (HIPAA).
